Job DetailsJob Location: Fort Scott, KS 66701Salary Range: $42.00 - $42.00 HourlyBASIC FUNCTION:    Physical Therapist Assistant evaluate, plan and provide therapeutic intervention as it relates to physical deficits in individuals with traumatic brain injury.     SPECIFIC DUTIES/RESPONSIBILITIES: •    Responsible for learning and implementing all aspects of Minds Matter LLC philosophical principles. •    Assists individuals to identify their needs and develop appropriate compensation techniques to accommodate the consequences of their brain injury.   •    Assessment-obtain pertinent data necessary for identification and/ or treatment of those in need of skilled intervention. •    Information may be accessed through medical records, observation and interviews. •    May contribute to the assessment and reassessment process under the supervision of the Physical Therapist (RPT). •    Treatment: Under the direction of the Physical Therapist the Physical Therapist Assistant will plan, direct and conduct programs to develop, improve and/or restore the performance of necessary functions to minimize debilitation.   •    Documentation: The Physical Therapist Assessment will keep/maintain treatment records, progress notes or other written plans as required and turn in as needed. •    Integrate employment, educational and independent living goals into therapy.   •    Work closely with the consumer and case manager to ascertain goals. •    Provide services only as designated on each individual consumer’s plan of care and /or physician orders. •    Work directly with consumers to improve skills in all aspects of their life goals. •    Maintain and turn in all documentation at assigned times. •    Educate families about the consequences of brain injury and communicate with them about goals and progress as needed. •    Attend all trainings and workshops to maintain ability to provide the most up to date, effective and evidence-based practice. •    Provide effective documentation that is legible, timely and appropriate to services provided to consumers. •    Be aware of departmental goals and objectives and take an active role in helping the department to attain goals. •    Schedule supervision meeting with therapy coordinator or physical therapist bi-monthly or as needed to review individual cases and departmental objectives/needs. •    Make initial contact within 48 hours of receipt of referral to schedule evaluation, which must be completed within 7 days of receipt of referral.   •    Provide case managers with quarterly summaries related to goal attainment. •    Meet all policy and procedure requirements of Minds Matter LLC on time including completion of timesheets, mileage, quarterly summaries, and other reports. •    Establish and maintain a professional rapport with all consumers, co-workers, referral agents and representatives of other community agencies and services, including attendance at planning meetings, etc. •    Maintain confidentiality of consumers medical records or services according to HIPAA regulations. •    Promote public relations throughout the agency and community for the program. •    Demonstrate a willingness to serve on agency and interagency task groups as requested. •    Provide care and maintenance for Minds Matter LLC issued computer if assigned. •    Perform other duties as assigned.   SUPERVISORY RESPONSIBILITY: None   QualificationsMINIMUM EDUCATION AND EXPERIENCE: •    High School or GED required with one year or more years of related experience preferred.  •    Associate’s or bachelor’s degree preferred   REQUIRED SKILLS AND ABILITIES: •    Proficient knowledge of Microsoft Office specifically Excel •    Excellent written, verbal communication skills •    Must be organized, possess problem solving skills, and be able to multitask •    Must have advanced analytical skills •    High degree of sensitivity toward all people •    Ability to effectively plan, coordinate and assist with intake process and systems •    Attention to detail   WORK LOCATION: Home and community  All duties, skills and abilities are considered essential unless otherwise noted.  This job description is not an employment contract.  This position may be required to perform other duties as assigned.
