Key Responsibilities

  • Conduct comprehensive biopsychosocial assessments and develop individualized service plans.

  • Provide outreach, service coordination, advocacy, and supportive case management in community settings including homes, shelters, and agency offices.

  • Link individuals to medical, behavioral health, educational, vocational, peer, and self-help resources.

  • Coordinate care among multiple service providers to promote recovery and optimal self-sufficiency.

  • Utilize trauma-informed and harm-reduction approaches in service delivery.

  • Provide crisis intervention for assigned and non-assigned clients when needed.

  • Facilitate discharge planning, aftercare coordination, and follow-up services.

  • Maintain accurate, timely, and legible documentation in narrative and data formats consistent with agency standards.

  • Develop and maintain collaborative relationships with colleagues and community partners.

  • Participate in supervision and ongoing professional development, including cultural competency growth.

  • Protect client privacy and confidentiality in accordance with agency policy and applicable regulations.

  • Perform additional related duties as assigned.


Qualifications

  • Bachelor’s degree in social work, psychology, or related human services field from an accredited college or university.

  • Experience in case management, behavioral health, or related services preferred (internship and volunteer experience considered).

  • Valid Pennsylvania driver’s license, reliable transportation, and appropriate insurance required.

  • Ability to obtain and maintain required clearances (FBI/Fingerprinting, PA Criminal, PA Child Abuse) and pass a drug screen.

  • Computer proficiency in Microsoft Office Suite; experience with electronic health records preferred.
