The Outreach Associate is responsible for developing relationships with members of the community and scheduling interested and eligible prospects for first appointments (“a Welcome Visit”), through hosting and attending various types of events. At these events, Outreach Associates will engage Medicare-eligible seniors in the community with the ultimate goal of having those individuals become patients of Oak Street Health, if they’re interested. Outreach Associates will work very closely with their Outreach Director/Outreach Manager, Community Relationship Manager and Outreach Executive(s) to help drive new patient growth locally in their assigned territory. On a daily basis, Outreach Associates will be the ‘face of OSH’ in their assigned territory and drive valuable local growth for OSH centers. Core Responsibilities include generating high-quality leads by engaging Medicare-eligible seniors through event execution at multiple locations, scheduling first appointments with interested and eligible prospects, maintaining knowledge of internal sales systems like Salesforce, collaborating with Community Relationship Manager, Outreach Executive, and Outreach Manager/Director to drive lead generation, and working with OSH central growth and call center teams as needed. They develop strong relationships with prospects, capture and record customer information and sales activity into CRM systems, act as point of contact for central call center team questions, resolve concerns and complaints, and perform other duties as assigned. The role requires ability to connect and influence community members, comfort with sales goals and results tracking, teamwork, results orientation, basic computer skills, experience with CRM systems (ideally Salesforce), sales or healthcare experience, reliable transportation, ability to lift 25-45 lbs, occasional work outside standard hours, fluency in Spanish, Polish, Russian or other community languages where necessary, and US work authorization.
