About Sentido Health

Sentido Health is a boutique home health solutions company founded on a deeply personal mission — to support medically complex patients and their families with the heartfelt care we'd give our own. We offer the product selection and clinical resources of large national organizations, delivered with the individualized approach only a boutique provider can. We consider our patients family, and our team is built on the belief that every person deserves more than a delivery — they deserve a partner.

The Opportunity

We're not just offering a job — we're offering a meaningful career in a mission-driven environment.

As an Account Manager, you'll play a critical role in expanding access to care for medically fragile pediatric patients while strengthening relationships with our referral sources and community partners. You'll manage territory-based accounts, drive referral growth, support patient documentation and authorization processes, and serve as the liaison between referral sources, patients, and internal clinical teams — collaborating closely with Area Managers and Patient Care Coordinators to ensure timely service, territory profitability, and exceptional patient satisfaction.

You'll join a collaborative, high-performing team that values growth, accountability, and excellence. We are searching for motivated professionals who are driven to succeed, thrive in a relationship-based sales environment, and are deeply committed to improving patient outcomes.

What You'll Do

  • Own your assigned territory and referral sources to support patient growth and service line expansion
  • Work shoulder to shoulder with internal teams to move pending patients through documentation and authorization processes
  • Track and work pending CMN documentation, reporting results to supervisors as requested
  • Take full ownership of your outlined monthly revenue and territory performance targets
  • Spot and develop new business opportunities within designated accounts and call points
  • Serve as the primary liaison and representative of Sentido Health to referral sources, physicians, case managers, and community partners
  • Conduct face-to-face and virtual meetings within your territory to strengthen relationships and ensure service excellence
  • Build and maintain weekly call routes and schedules that keep your accounts consistently covered
  • Facilitate patient education and help resolve service concerns in collaboration with internal staff
  • Keep documentation in Brightree and other internal systems accurate and timely
  • Ensure compliance with HIPAA and company policies in all communications and handling of protected health information
  • Manage territory-level operational expenses within approved budget guidelines
  • Maintain on-call participation as required to ensure appropriate territory coverage
  • Complete emergency deliveries or home visits as needed

What You Bring

  • The ability to recognize and respond to the needs and concerns of patients, families, and referral sources
  • Strong oral and written communication skills
  • Professional telephone and email etiquette
  • A self-starter mentality — you work independently in a fast-paced, goal-oriented environment
  • Strong time management, prioritization, and organizational skills
  • Effective problem-solving and decision-making abilities
  • The ability to multitask while maintaining high levels of customer service and responsiveness
  • Demonstrated professionalism, integrity, accountability, and competitive drive
  • The ability to educate patients and family members on services and equipment as applicable
  • Ability to meet established daily appointment and territory conversion targets
  • Effective inventory management within assigned territory

  • Bachelor's degree preferred or equivalent combination of education and experience. Bilingual skills preferred.
