An Associate Director of Housing contributes to AHF by providing managerial oversight of Care Management programs and contracts. This position requires astrong understanding of Ryan White program, HOPWA, HUD and a through knowledge of program guidelines, service deliverables and available resources. You will collaborate with the Regional team, the Leadership Team, and Healthcare Center staff to ensure that all staff understand operational processes, deliverables, and performance goals of the Care Management Division and Ryan White program. In this role, you will provide guidance and operational support to your team, monitor and analyze productivity and performance reports, and identify trends, best practices, and challenges. By conducting regular one on one meetings with Managers and staff, you will gain insight into barriers and challenges. With a solution-oriented focus, you will foster collaboration across AHF to ensure seamless service delivery and adherence to organizational standards. An  Associate Director of Housing, will be supporting managers, staff and clients referred through Housing continuum of care, including individuals who are unhoused and seeking permanent housing, as well as those currently housed who require ongoing support to maintain stability. Serving within the Permanent Supportive Housing and the different housing programs available per contract. The Associate Director of Housing will facilitates access to housing and supportive services by identifying client and staff needs, developing individualized service plans, and conducting regular follow-ups to ensure progress and retention. They work with partner agencies to coordinate care, remove barriers to housing, and provide targeted referrals for behavioral health, medical, financial, and social services.

An Associate Director of Housing enhances the reputation of AHF by providing consistent, field-based support to residents enrolled across all available housing programs. They oversee each program to ensure compliance with approved caseloads, delivery of required services, and completion of mandated monthly in-home visits. Through this oversight, the Associate Director monitors housing stability, identifies emerging client needs, and ensures timely referrals to appropriate resources. The role includes regular participation in coordination meetings with Housing staff and partner agencies to align care strategies and maintain adherence to Housing and HOPWA standards of care. In addition to direct client support, the Associate Director cultivates relationships with external service providers, identifying opportunities to establish formal partnerships through memorandums of understanding (MOUs).

The Associate Director of Housing will work as a part of a close-knit team to support managers, housing staff and residents in programs such as TBRA, PHP, STRMU, and HUD by accepting referrals, completing client enrollments, and developing individualized care plans. They will facilitate access to transportation and provide targeted referrals to behavioral health, medical, and social services, with a strong emphasis on connecting clients to behavioral health support through Housing Forward’s partner agency, NTBHA. The Housing Care Manager - HF will become familiar with Housing Forward’s intake, recertification, and exit processes, and will document all client encounters and actions taken on the client’s behalf. They will also contribute to a dynamic resource database and help ensure services are equitable, inclusive, and tailored to individual needs. Through active coordination with service providers and participation in regular meetings with Housing/HOPWA/HUD and partner agencies, the Associate Director of Housing Services helps maintain alignment with continuum standards of care and promotes long-term housing stability. Additionally, this position provides oversight, guidance, and support to local Managers in each region to eliminate barriers to guarantee a seamless client experience.

Licenses and Certifications: BS/BA in Social Work, or other social service field with two (2) years of direct service delivery. Familiarity with housing case management. Familiarity with housing systems and procedures. Ability to travel to the regions up to 50% of the time.
