Primary Responsibilities:
- Provide individual and group counseling to assigned clients; collaborate with Clinical Supervisor as needed.Â
- Establish a treatment plan for each individual. Â Â
- Provide related information and referrals to community-based resources including Fuel Assistance, Food Stamps, food pantries, and other services that can assist the client in the maintenance of sustainable housing.Â
- Coordinate all services required by the treatment plan. Â
- Ensure clients comply with program guidelines and complete assigned daily chores.Â
- Maintain complete and accurate client files and records; complete all required paperwork accurately and timely.Â
- Work collaboratively with other SMOC staff on housing and employment search and stability as indicated in the treatment plan. Â
- Maintain weekly contact and reassess, evaluate and document adherence to the treatment plan regularly. Â Â
- Support clients in their continued economic development planning. Â Â
- Provide monthly communication where necessary and mediate any conflict. Â Advocate in Housing Court. Â Â
- Maintain up to date case notes, telephone contact log and referrals to community-based services. Â Enter all client data into the required database(s) daily. Â Â
- Participate as a member of the Case Management team, attend all team meetings. Â Â
- Conduct groups and workshops related to the clients' needs as appropriate.Â
- Assist clients in arranging appointments and transportation.Â
- Engage all clients by understanding and addressing their needs whether within or outside the scope of work.Â
- Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.Â
- Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.Â
- Ensure compliance with program/department, agency and/or funder requirements, as well as SMOC policies & procedures.Â
- Other duties as assigned. Â
Knowledge and Skill Requirements:
- This role requires working with female clients. Due to privacy and safety requirements this position is designated as female only.Â
- Minimum of one year experience in Human Services or related field. Â
- Certification or license in substance abuse counseling (CADAC or LADAC) or at minimum enrolled in a certificate program.Â
- If applicable, a minimum of 3 years continuous sobriety.Â
- Minimum one year assessment, advocacy and case management skills. Â
- Sensitivity to low-income people of diverse backgrounds; excellent listening skills.Â
- Ability to work independently and collaboratively. Â
- Strong written and verbal communication skills.Â
- Must have valid driver’s license, reliable transportation and meet insurance standards.  Â
Organizational Relationship:Â
- Directly reports to Clinical Supervisor and Program Director. Â Â Â

Physical Requirement:Â
- Ability to drive a vehicleÂ
- Must be able to ascend/descend stairs in residences
Working Conditions: Some travel via employee’s personal vehicle is required. A successful background check is required.
Tuesday through Saturday: 8:00AM - 4:00PM. Includes a 1-hour unpaid lunch break.
35 Hours per week.
