Summary

Under the direction of the Asst. Director the Program Navigator assists in the coordination and implementation of long-term services and support in the community. At CCC all duties are performed in a manner that fosters the achievement of the organization’s mission.


Essential Functions Ā 

1. Provides Referral Screening for all new potential clients of Public Programs supported by CCC.Ā 

2. Provides telephonic communication with client and/or the referral source to provide education and program navigation.

3. Schedules initial assessmentsĀ 

4. Follows, supports and assists pending client's post-assessment to assist with timely activation onto appropriate program/s. This will include home visits as needed.

5. Participates in and works effectively as part of an interdisciplinary team.

6. Assist in mentoring new staff as needed.

Requirements

Education:

High School Diploma required; Associate Degree in Health or Human Service preferredĀ 


Experience:

Ā· For High School graduates, at least two years’ experience in a human service setting preferred.

Ā· For attained Associate Degrees, at least one year experience in a human service setting, or equivalent experience preferred.

Ā· Project management and organizational skills.

Ā· Strong verbal and written communication skills.

Ā· Knowledge of community resources preferred.

Ā· Ability to maintain confidentiality of client and company information.

Ā· Ability to understand and articulate multiple programs.

Ā· Working knowledge of Connexus.


Other:

Ā· Reliable transportation, valid driver’s license and current automobile insurance.
