As a member of the Interdisciplinary Team, the RN Case Manager facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.   The RN Case Manager is also responsible for promoting the philosophy of hospice, which redefines hope by focusing on quality of life and connection with loved ones during a patient’s final days, rather than on curing a disease.   Why Hinds LIfeCare:   A mission-driven organization and a pioneer in hospice care, Hinds LIfeCare seeks to uphold the dignity and ease the suffering of the terminally ill while supporting their loved ones and those who are grieving. One year experience in acute care facility or community health agency, (hospice experience a plus). Current RN Licensure and CPR Certification. Valid driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment. Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.) Availability to work any of the following schedules: Monday - Friday, Tuesday - Saturday, or Sunday - Thursday.
