What you bring to SBHG:
- Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR
- Bachelor’s degree in a social science field plus six (6) months experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Driver’s License required and must meet Agency driver eligibility criteria.
How you will make a difference:
- Provides services at the TAY Center, in the community, and in TAY’s homes. Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAY’s family members in personal vehicle.
- Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
- Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
- Monitors the clients’ progress toward goals outlined in their treatment plans. Informs primary therapist of progress.
- Develops resource directories related to housing and social support resources.
- Assists clients with securing housing.
