We are seeking a full-time Physical Therapist to provide care for patients in both inpatient and outpatient settings. In this role, you will work with individuals experiencing a wide range of conditions at Emplify Health by Gundersen, Moundview Memorial Hospital & Clinics and the Extended Care Center in Friendship, WI.

What you will do/job specifics

• Full-time 40 hours/week

• 8 hour shifts, 7am-3:30pm and 8am-4:30pm

• Occasional coverage on weekends

• Work closely with patients to assess their physical and psychosocial needs.

• Planning, development, and implementation of multidisciplinary plan of care

• Evaluation of patients' responses to care.

• Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries.

What’s Required/What you need

• Graduate from an accredited program for Physical Therapy

• Current licensure as a Physical Therapist in the State of WI

• CPR Certification (or ability to obtain within 3 months)
