Liberty Resources Integrated Health Care is seeking a compassionate, licensed Community-Based Therapist to join our expanding Youth Assertive Community Treatment (Youth ACT) team. Position Summary
The Community-Based Therapist provides intensive, evidence-based behavioral health treatment and care coordination for youth experiencing Serious Emotional Disturbance (SED) and complex behavioral health needs. As a member of the Youth ACT multidisciplinary team, you will deliver services in homes, schools, and community settings using trauma-informed, family-driven, and youth-guided practices that promote recovery, stability, and long-term success.
Key Responsibilities
- Conduct comprehensive behavioral health assessments and develop individualized treatment plans.
- Provide evidence-based individual, family, and group therapy in home, school, and community settings.
- Deliver crisis intervention, risk assessments, safety planning, and ongoing clinical support.
- Collaborate with psychiatrists, nurses, care managers, schools, healthcare providers, and community agencies to coordinate services.
- Promote youth engagement, family participation, skill development, and successful community integration.
- Participate in multidisciplinary treatment team meetings and shared caseload management.
- Complete timely, accurate clinical documentation in compliance with OMH, Medicaid, HIPAA, and agency standards.
- Participate in required trainings, supervision, quality improvement initiatives, and fidelity monitoring.
- Participate in rotating after-hours on-call coverage as required.
- Perform other related duties as assigned.
Qualifications
Required
- Master's degree in Social Work, Mental Health Counseling, Marriage & Family Therapy, Psychology, or a related behavioral health field.
- Current New York State license including one of the following:
- LCSW
- LMSW
- LMHC
- LMFT
- Licensed Psychologist
- Experience working with children, adolescents, and families experiencing behavioral health challenges.
- Strong assessment, treatment planning, crisis intervention, and care coordination skills.
- Knowledge of trauma-informed care and evidence-based treatment approaches.
- Excellent communication, documentation, and organizational skills.
- Valid New York State Driver's License and reliable transportation.
Preferred
- Previous Youth ACT, ACT, Home-Based Therapy, or intensive community behavioral health experience.
- Experience serving youth with Serious Emotional Disturbance (SED), trauma, co-occurring disorders, and complex behavioral health needs.
- Knowledge of OMH regulations, Medicaid documentation requirements, and electronic health records.
- Current CANS-NY Certification or ability to obtain required certification.
- Proficiency with Microsoft Office 365 and telehealth platforms.
