About the Role:
Talin Home Healthcare LLC is looking for multiple compassionate and dedicated Home Health Nurses to serve patients across different Northern Virginia counties area. Responsibilities:
- Provide skilled nursing care to patients in their homes across the Northern Virginia area
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other clinical treatments as ordered
- Monitor and document patient progress, vital signs, and changes in condition
- Educate patients and family members on disease management, medications, and home care techniques
- Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
- Active RN license in the state of Virginia
- Oasis SOC Experience preferred and a plus.
- Previous home health, med-surg, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and valid driver's license for travel throughout the NOVA region
- Ability to work independently and manage a flexible patient caseload
- Compassionate, patient-centered approach to care
- Must commit to working a minimum of 2 days a week 1 day must be a Monday or a Tuesday.
