Community Supervision

JOB SUMMARY:

Provides community case support for participants active in an accountability court program. Works directly with individuals diagnosed with substance use and/or mental health disorders involved in the criminal justice system. Responsibilities include home visits, transportation assistance, vocational support, ensuring attendance at treatment and court sessions, and conducting medication counts. Reports to the Program Coordinator.

 ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

• • Provide limited para‑professional social services to participants.
  • Update and maintain accurate information on participants' healthcare and social backgrounds.
  • Prepare, update, and maintain clear and concise notes in participant charts per departmental policy.
  • Assist in monitoring participant progress toward treatment goals.
  • Provide referrals to facilitate continuity of care.
  • Teach daily living skills and document participant progress.
  • Assist participants in locating and utilizing community resources, including legal, medical, financial, and referral services.
  • Provide appropriate emotional support and assist in resolving minor problems.
  • Help participants perform community integration activities.
  • Collect and review participant assignments such as self-help meeting logs, job verifications, and sanction receipts.
  • Assist with fee collection and payment management in electronic records system.
  • Conduct medication counts and report compliance issues to the team.
  • Assist participants in accessing transportation.
  • Support participants in seeking vocational opportunities
  • Attend and participate in treatment team meetings and court sessions.
  • Assist in planning and implementing effective activities for participants.
  • Assist with drug testing collection as needed.
  • Provide administrative support to treatment staff.
  • Provide appropriate input to develop and maintain a network of community contacts.
  • Prepare and submit required reports and documentation as requested.
  • Maintain confidentiality of participant and office information as required by Federal law.
  • Regular and predictable attendance is required.
  • Perform other related duties as assigned.

 MINIMUM QUALIFICATIONS REQUIRED:

Education and Experience

One year full-time (or equivalent part‑time) experience in social services delivery (case management, outreach, prevention/education, etc.) OR one year full‑time experience performing direct care activities, basic nursing procedures, or client training at the level of a health services technician.

Associate’s degree in Social Work, Psychology, Nursing, Education, or related field required.

Must be able to work in a fast‑paced court environment with limited supervision.

Must have working knowledge of Microsoft Office Suite.

Licenses and Certifications

Valid Georgia Driver’s License

• Apply knowledge of mental health and substance use disorders and effective treatment approaches.
• Operate a personal computer, software applications, internet tools, multimedia programs, and a calculator.
• Develop and maintain effective working relationships with departments and the public.
• Communicate clearly
• Maintain confidentiality with sensitive information.

Physical Ability

Frequently required to move about the office and transport items up to 10 lbs.

Occasionally required to bend, reach overhead, push/pull, twist, and perform repetitive motions; may be on‑call and work irregular hours.

Sensory Requirements

Regularly requires effective verbal and written communication, hand‑eye coordination, and fine motor skills.
