Social Worker Opportunity:

• Full-Time | Permanent Position | Monday-Friday Schedule
  

• Social workers must provide direct support and case management services to assigned patients and families
  

• Coordinate resources and services to support positive patient outcomes
  

• Advocate for patients while helping navigate social, emotional, and community-based needs
  

Social Worker Qualifications:

• Current Licensed Social Worker (LSW) license required
  

• LICSW candidates are highly preferred
  

• Hospital or healthcare experience preferred
