Qualifications

• Minimum of 1 year current LPN experience in an acute care setting (home care experience preferred)

• Authorized to work in the United States

• Valid NYS LPN license

• Current BLS certification

• Valid driver’s license (preferred)

• Physical exam completed within the past year

• QuantiFERON within the past year or chest X-ray within the past five years

• MMR immunizations within the past ten years
