- Pay: $60/hour first 90 days → $120,000 salary after ramp
- Schedule: Minimum 32 hours/week availability

What you’ll do

Deliver in-home primary care visits for adult patients

Conduct physical exams and diagnose conditions

Prescribe medications and adjust treatment plans

Build personalized care plans for chronic conditions

Coordinate care with specialists and healthcare partners

Complete follow-ups after hospital discharge (TCM visits)

Document visits and communicate with care team
Must-haves
-
MD, DO, Nurse Practitioner, or Physician Assistant license
-
Licensed in Indiana

Master’s degree in Nursing or Physician Assistant Studies

Strong clinical assessment and diagnosis skills

Authorized to work in the United States

Comfortable working independently in patient homes

Experience in in-home or mobile healthcare

Wound care certification or experience

Experience with transitional care or chronic care management
