Job Description

You'll provide hands-on nursing care for older adults in Georgetown, Ohio, supporting medication administration, vital signs, documentation, family education, and interdisciplinary care planning in a long-term care environment.

You'll apply your LPN skills to provide direct nursing care, collaborate with an interdisciplinary team, educate families, and support individualized care plans while promoting resident comfort, dignity, and quality of life.

Schedule: PRN

Key Responsibilities

  • Provide hands-on nursing care for older adult patients in a long-term care environment.
  • Take vital signs, document care, and maintain clear patient records.
  • Administer prescribed medications, monitor reactions, and assist providers during exams and treatments.
  • Support patients with activities of daily living, including dressing, bathing, eating, and comfort measures while promoting quality of life and providing end-of-life care as appropriate.
  • Educate families and participate in care planning with the medical team.
  • Follow safety policies, care procedures, and assigned nursing workflows.

Qualifications

Requirements

  • High school diploma or GED.
  • Graduate of an accredited LPN program.
  • Active and unrestricted Ohio LPN license.
  • Current BLS from the American Heart Association or Red Cross.
  • 6 months of LPN experience.
