Occasional weekend coverage required. GENERAL SUMMARY Under the direction and supervision of the LCSW Field Instructor and/or designee(s), the Social Worker provides case management and social work services to patients and families within assigned areas in order to meet identified psychological needs, enhance problem solving and coping capacities, and to facilitate timely discharge by arranging post-discharge services. The Social Worker provides, under the direction and supervision of the LCSW Field Instructor, psychosocial assessment, supportive counseling, crisis intervention, and financial planning and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning for to all patients. The Social Worker works with the multidisciplinary team to ensure safe transitions of care. Working closely with medical staff and nursing units in this effort, effective and efficient utilization is accomplished. PRINCIPAL DUTIES AND RESPONSIBILITIES 1. Participate in patient progression throughout the continuum to achieve desired outcomes and organizational goals while promoting continuity of care, collaborative practice, and appropriate utilization of resources. 2. Collaborate with all members of the interdisciplinary team, as well as external customers and payor sources, to effectively facilitate care coordination and care delivery for our patients. 3. Monitor patient progress, intervening as directed to ensure the plan of care and services provided are patient focused, efficient, cost effective, and directed toward maximizing clinical outcomes. 4. Identify and assist in resolving delays and obstacles to discharge. 5. Meet with patients/families as directed to assess needs and develop a patient-centered discharge plan, collaborating and communicating with the interdisciplinary team in all phases of the discharge planning process. 6. Participate in the referral process for home health, Hospice, durable medical equipment and supplies, nursing home placement, and Swing Bed placement. 7. Identify at risk populations using approved screening tools, following established reporting procedures. 8. Monitor length of stay and ancillary resource used, taking action to achieve continuous improvement in both areas. 9. Seek consultation from appropriate disciplines/departments as required to expedite care and facilitate timely discharge. 10. Support the collection of appropriate resource utilization data and clinical outcomes and process data for specific performance and/or outcome indicators. 11. Demonstrate positive and professional written, verbal, and nonverbal communication skills. 12. Demonstrates professional and effective interpersonal skills in promoting constructive problem solving utilizing critical thinking and innovative approaches. 13. Other duties as assigned. Work requires communication in English in order to communicate, understand verbal instructions, ask questions, and give direction to and from staff, patients, visitors and family members whether on the phone or in person. 4. Work requires ability to function well under pressure and conditions of frequent interruptions. 5. Work requires ability to use a keyboard to enter and transform words or data on a daily basis. Ability to communicate professionally and succinctly in writing. 6. Visual activity necessary to observe patient, obtain information, and use documentation. 7. Auditory acuity to hear patient/family/other for the purpose of communication. REPORTING RELATIONSHIP Director of Quality and Case Management, indirectly to the Executive Director of Quality Assurance. QualificationsEDUCATION, KNOWLEDGE AND ABILITIES REQUIRED 1. Student of Social Work currently enrolled in an accredited advanced degree program and in good academic standing with sponsoring institution. 2. Understanding of social work ethics and values. 3. Computer proficiency (Microsoft Word, Excel, patient databases). 4. Analytical abilities and organizational skill to assess patient needs, prepare care and discharge plans, and find solutions to difficult human problems. 5. Emotional stability to deal with high stress level associated with working with acutely ill patients and family psychosocial problems and maintaining effective working relationships with peers and physicians. 6. BLS certification preferred. INFECTION EXPOSURE RISK LEVEL CATEGORY 2- LOW RISK: Position contains tasks that involve no exposure to blood, body fluids, or tissues, but employment may require performing unplanned tasks that do involve exposure. WORKING CONDITIONS 1. Works in a normal office or patient care environment where there are relatively few discomforts due to dust, dirt, noise, and the like. 2. Works with patients and may be exposed to contagious diseases of infectious material, but potential for personal harm and injury is limited when proper safety and health precautions and equipment are used.
