Job DetailsJob Location: GIBSON CITY, IL 60936Position Type: Full TimeSalary Range: $32.00 - $48.00 HourlyJOB TITLE:    Diabetes Educator (RN) DEPARTMENT:  OPC Specialty Clinic HOURS & SHIFT REQUIREMENTS:   Full time, 80 hours a pay period, Monday – Friday with an occasional weekend or evening for community events. GENERAL SUMMARY:  The Diabetes Care & Education Specialist provides education, support, and guidance to individuals with diabetes and their families to promote optimal self-management and improve health outcomes. This role involves assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure comprehensive diabetes care. GIBSON AREA HOSPITAL & HEALTH SERVICES MISSION STATEMENT To provide personalized, professional healthcare services to the residents of the Communities we serve. PRINCIPLE DUTIES AND RESPONSIBILITIES Conduct assessments of patients’ knowledge, skills, and readiness for diabetes self-management. Develop and deliver individualized diabetes education covering topics such as nutrition, blood glucose monitoring, medication management, diabetes-related technologies, lifestyle modification, and complication prevention.   Provide ongoing support, counseling, and follow-up to encourage adherence and behavioral changes.  Monitor patient progress and adjust education plans as needed. Coordinate centralized continuous glucose monitoring (CGM) workflows, including device onboarding, data integration, and patient follow-up. Provide remote patient monitoring of CGM data to identify trends, support timely interventions, and communicate recommendations to the care team. Serve as a resource for insulin pump and automated insulin delivery (AID) systems, including patient education, troubleshooting, and therapy optimization. Collaborate with healthcare providers, including physicians, nurses, dietitians, and pharmacists, to ensure coordinated care. Maintain accurate documentation of education sessions and patient outcomes in accordance with regulatory standards. Stay current with advances in diabetes care, treatment guidelines, and patient education strategies. Participate in community outreach, workshops, and support groups as needed. PHYSICAL REQUIREMENTS Excellent, visual, hearing, speech/articulation abilities. Manual dexterity to use computer to communicate, investigate and educate. Work requires ability to sit at a computer for long periods of time (2-3 hours) REPORTING RELATIONSHIP The DCES reports directly to the DSMES Quality Coordinator.   QualificationsEDUCATION, KNOWLEDGE AND ABILITIES REQUIRED: Bachelor’s degree in Nursing Certification in Diabetes Education (CDCES) preferred or willingness to obtain Knowledge of diabetes pathophysiology, pharmacology, and self-management strategies Strong communication, teaching, and counseling skills Patient-centered approach with empathy and cultural sensitivity Ability to motivate and empower patients toward self-management goals Strong critical thinking and problem-solving skills in complex patient situations Ability to work both independently and collaboratively within a multidisciplinary team Excellent organizational skills with strong attention to detail Proficiency with electronic medical records (EMR) and patient education tools INFECTION EXPOSURE RISK LEVEL  CATEGORY 2 – MODERATE RISK - The job potentially exposes the employee to blood, body fluids and tissues.  The Hepatitis B vaccine is recommended. WORKING CONDITIONS Sitting at a desk, typing and use of computer, for long periods of time Occasional travel to meetings, educational offerings outside the facility Occasionally works in patient care areas where there are few discomforts due to the environmental temperature, dust, dirt, and noise. Occasionally faces extremes in temperatures when working in community setting. Works with community members and patients and may be exposed to contagious diseases and infections materials, but potential for personal harm and injury is limited when proper safety and health precautions and equipment are used.
