What You Will Do: Scheduling required programs/visits for all Home Care Companion, Home Health Aide, Certified Nursing Assistants Effectively manages and communicates all changes in resident/client needs to staff as updates are made to the contract and plan of care Effectively maintains client chart/documentation and filing practices as per company guidelines and best practices Displays confidence when scheduling difficult or interdepartmental requests Recognized by customers as knowledgeable and helpful in scheduling procedures Ensures schedules are communicated/distributed to staff Manages on-call/after-hours needs as assigned Maintains positive relationships and rapport with co-workers, residents, family members and community personnel Liaison between client and caregiver Contract meetings with family and client Semiannual chart audits Maintain compliance with employee files Reports to Manager and Director when there is a change in clients status or service requirement Other duties as assigned by Manager and Director   What You Will Bring: Must have experience with Microsoft Office, technology and social media Multi-tasking is a must, excellent communication and interpersonal skills are required   Why Choose Us?
