Position Summary

The Physical Therapist is responsible for providing high-quality evaluation and treatment services to patients in accordance with physician orders and established clinical standards. This role involves developing individualized care plans, monitoring patient progress, and collaborating with multidisciplinary teams to achieve optimal outcomes.


Essential Responsibilities

  • Perform physical therapy evaluations and treatments based on physician referrals and patient needs.

  • Conduct comprehensive assessments using appropriate tools and review patient medical records for diagnosis, history, precautions, and contraindications.

  • Develop measurable goals and individualized treatment plans to maximize independence and function.

  • Regularly reassess patient progress, modify goals as needed, and document all care accurately and promptly.

  • Communicate effectively with physicians, patients, families, and other members of the care team regarding patient status and treatment outcomes.

  • Recommend and assist with adaptive or assistive equipment to enhance mobility and functionality.

  • Maintain compliance with documentation, billing, and reporting standards required by company policy, state, and federal regulations.

  • Adhere to ethical and professional standards, including HIPAA, company compliance programs, and applicable laws.

  • Supervise Physical Therapist Assistants and Aides in accordance with state practice acts and organizational policies.

  • Participate in staff meetings, professional development activities, and continuing education to maintain and enhance clinical skills.


Qualifications

  • Current Physical Therapist license in the state of practice (Compact License accepted).

  • Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.

  • Current CPR certification (preferred).

  • Strong clinical reasoning, documentation, and communication skills.

  • Ability to work independently and as part of an interdisciplinary team.
