Salary: Starting at $45000 / Annually - depending on education / experience

 

Job Summary:

The primary purpose of this position is to conduct investigations in support of Woods’ commitment to protect the health, safety and rights of the individuals it serves.  Investigators will interview individuals, staff and witnesses, review documentation and collect evidence.  They will prepare written reports and document findings, Administrative Review Committee (ARC) conclusions, and Human Resources determinations resulting from the investigations.  The Investigator will work with Woods’ Compliance and Licensing Department, Legal Department, Human Resources Department and Management to share results of the investigations and respond to inquiries from regulatory and funding agencies and law enforcement.


Responsibilities

  • Investigates incidents as mandated by regulations which can include locations throughout Woods Services and its affiliates;
  • Ensures all policies and procedures pertaining to investigations are followed;
  • Plans the scope and direction of the investigation in consultation with the Initial Reporter;
  • Conducts the investigation in a timely, thorough, and objective manner in accordance with the guidelines provided within the Pennsylvania Certified Investigator Training Program and Woods policies;
  • Interviews individuals, staff and witnesses;
  • Collects, constructs, organizes and analyzes physical and testimonial evidence;
  • Reviews Incident Reports, Behavior Reports, Video if available, Medical, Residential and other documents, as necessary;
  • Applies Woods Policies and Procedures and Regulatory Standards to investigative situations, organize work, meet deadlines, and render findings for review by the Administrative Review Committee and Human Resources;
  • Writes an investigative report that includes a summary of the evidence, notifications made, documentary evidence collected, statements, ARC conclusions and HR determinations;
  • Inputs information into the HCSIS or EIM database in accordance with regulatory requirements;
  • Communicates investigation results to funding and regulatory agencies;
  • Responds to any investigation follow up questions requested by licensing and funding agencies and participate in the plan of correction process as needed;
  • Attends Depositions and Hearings if required and assist the Chief Legal Officer (CLO) and outside counsel with respect to all hearings and in litigated matters;
  • Participates as a member of Incident Review Committee(s);
  • Maintains confidentiality of the investigation and Peer Review protection;
  • Maintains positive working relationships with all management staff and co-workers at all times.
  • Complies with all record retention polices as they pertain to Incident Investigation in compliance with State, Regulatory, Funding and Managed Care entities; and
  • Other duties as assigned.

Qualifications

  • Bachelor's Degree in the Human, Social, Behavioral, Nursing or Criminal Sciences and two years Human Service, Law Enforcement or investigative experience.
  • Must be a Pennsylvania Certified Investigator, or obtain certification within 3 months, and maintain certification by completing continuing education requirements. 
  • Experience working with Intellectual or Developmental Disability (IDD) or Mental Health population required.  Ability to be on-call during nights/weekends/holidays as needed. Must hold a valid driver’s license.
