Job DetailsJob Location: Goshen, NY 10924Position Type: Full TimeSalary Range: $55.00 - $55.00 HourlyJob Shift: DayPOSITION SUMMARY: The MAT Care Coordinator is a member of the integrated health team. Position is responsible for engaging and supporting patients receiving Medication Assisted Treatment (MAT). The MAT Coordinator works with the provider and the health care team to coordinate care resources and services to improve the quality and effectiveness of patient care. Collaborates and communicates with stakeholders regarding improvements, data entry, treatments assessments and the success of the MAT Program. POSITION REQUIRMENTS: HS Diploma Drug/alcohol experience preferred BLS/CPR certification. Ability to establish and maintain effective, courteous working relationships with patients and staff team members. Ability to work under pressure in a fast-paced environment. Ability to travel to ​​​ meetings outside of the service area. At least one year of experience in an ambulatory health care suite desired. Computer literacy in Word and Excel Corrections preferred Ability to obtain clearance to work in a correctional facility.   PERFORMANCE EXPECTATIONS: Performs key functions related to the success of the MAT program to include tracking of services provided. Ensures effective collaboration and communication between members of the healthcare team to support care coordination activities and goal attainment Provides post-discharge follow-up calls, acts as a liaison with specialty clinics, addresses and works to resolve patient concerns or barriers to treatment planadherence, and provides a link to appropriate community resources. Consults with providers and other team members as appropriate to resolve care issues. Documents all patient care activities; enter information into the appropriate system and maintain the patient registry. Maintains patient registry and regular data monitoring including data entry of patient data into outside platforms. Acknowledges patient’s rights and always maintains patient confidentiality and maintains documentation in compliance with agency, HIPAA, 42CFR, and funder standards Participate in grant funded activities specifically around MAT, including data entry (also in outside data platforms), data monitoring in coordination with data team All other duties as assigned QualificationsQualifications High School Diploma or equivalent required Prior experience working with individuals affected by substance use disorders, behavioral health, or drug/alcohol treatment preferred Experience in an ambulatory healthcare, care coordination, case management, or clinical support setting preferred Current BLS/CPR certification required or ability to obtain within a specified timeframe Strong interpersonal and communication skills with the ability to establish and maintain effective, respectful, and professional relationships with patients, providers, and interdisciplinary team members Demonstrated ability to work effectively in a fast-paced, high-pressure healthcare environment Proficient in basic computer applications, including Microsoft Word and Excel; experience with electronic health records and data entry systems preferred Experience working in correctional or institutional healthcare settings preferred Ability to travel to meetings or program-related activities outside of the primary service area as needed Ability to obtain and maintain required security clearance to work in a correctional facility Commitment to maintaining patient confidentiality and compliance with HIPAA, 42 CFR Part 2, agency policies, and applicable regulatory requirements
