Clinical Coordinator

The Clinical Coordinator supports the efficient coordination of participant care and clinical operations within the Program of All-Inclusive Care for the Elderly (PACE). This role serves as a central point of coordination for regulatory assessment scheduling, participant-specific supplies and equipment, oxygen management, participant communication, and clinical follow-up activities.

Working closely with the interdisciplinary team (IDT), clinical staff, Home Care Manager, Primary RNs, IDT Coordinator, participants, families, and external vendors; the Clinical Coordinator helps ensure that required services, assessments, supplies, and follow-up activities are completed accurately and within required timeframes.

This position primarily works in a clinical and office environment with extended periods of sitting, standing, and/or walking may occur. Regular communication with participants who have complex medical, cognitive, functional, and psychosocial needs will occur throughout the day. 

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:

  • Associates degree in healthcare related field preferred.
  • Minimum of 1 years’ experience in healthcare coordination, clinical operations, scheduling, or related healthcare support role required. 2 years’ experience preferred.
  • Experience working with older adults, medically complex individuals, home care, primary care or long-term care preferred.
  • Experience coordinating schedules, appointments, services, or supplies strongly preferred.
  • Colorado drivers license, adequate automobile insurance, and unlimited access to reliable transportation maintained throughout employment. 
  • Strong organizational and coordination skills with exceptional attention to detail.
  • Ability to manage multiple deadlines, priorities, and follow-up activities simultaneously.
  • Strong communication and relationship-building skills.
  • Ability to communicate effectively with participants, families, clinical staff, interdisciplinary team members, and external vendors.
  • Ability to anticipate needs and proactively address potential barriers.
  • Strong problem-solving and critical-thinking skills.
  • Ability to develop and maintain effective tracking systems and monitor completion of time-sensitive activities.
