Key ResponsibilitiesÂ
The Occupational Therapist administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.Â
In this role you will be responsible for:Â
- Evaluate patient's functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.Â
- Evaluates home environment for hazards and barriers to more independent livingÂ
- Participates in the development of the individualized plan of care including equipment needs
- Develops treatment program and establishes goals for improved functionÂ
- Communicates plan of care to referring physician and other organization professionals
- Teaches new skills or retrains patients in once familiar daily activities that have been lost due to illness or injury, in accordance with the agency’s policyÂ
- Maintains appropriate clinical records, progress notes, and reports to the physician or advanced practice practitioner any changes in the patient’s condition
- Submits records in accordance with the agency’s policyÂ
Required QualificationsÂ
- A graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy AssociationÂ
- 1+ years clinical experience as an Occupational Therapist
- Registered with the National Registration Examination of the American Occupational Therapy AssociationÂ
- Requires interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregiversÂ
- Knowledge of medical terminologyÂ
- Able to demonstrate problem-solving and conflict resolution skills, organizational skills, and attention to detailÂ
- Must have strong verbal and written communication skillsÂ
- Demonstrates ability to handle multiple priorities, documentation requirements and deadlinesÂ
- Reliable transportation and valid automobile insurance coverageÂ
- Sit, stand, bend, lift, and move intermittentlyÂ
- Ability to lift at least 35lbs and bear the weight of an average adult effectivelyÂ
Desired QualificationsÂ
- Home Health experience preferredÂ
- Familiarity with Home Care Home Base (HCHB)Â
