Palliative Care Manager (LMSW)  Full-Time | Grand Rapids, MI and Surrounding Areas
At Emmanuel Hospice, we understand the role that palliative care can offer in our community and we are excited to bring this service to life alongside our core hospice services. This role offers the rare opportunity to be a cornerstone in launching Emmanuel Hospice’s new palliative care program, shaping its vision, structure, and long term impact. As a key leader, you will guide the development of innovative care models that will meaningfully support patients who are facing serious illness and their families. Your expertise will directly influence how the program grows, evolves, and becomes an essential part of the Emmanuel Hospice continuum of care.
Reporting directly to the CEO, the Palliative Care Manager ensures the program reflects excellence, regulatory compliance, and the mission to support patients and families with serious illness.
This full-time, Salaried position offers a dynamic and rewarding experience for a social worker who thrives in diverse environments and enjoy being part of a collaborative, mission-driven team.
Key Responsibilities:
- Lead all program startup activities, including workflows, policies, and operational designÂ
- Manage program budget, financial performance, billing processes, and revenue cycleÂ
- Supervise and support palliative care staff in collaboration with the Medical DirectorÂ
- Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standardsÂ
- Support provider credentialing and payer contractingÂ
- Ensure efficient scheduling, referrals, documentation, and care transitionsÂ
- Drive program growth, customer service, and performance improvementÂ
- Deliver social work services during startupandas neededÂ
- Participate in the ManagerOnCallRotationfor hospice services
- Travel within the service area to patient homes and healthcare facilities
Qualifications:
- Master's degree in social workrequired; hospice or palliative care related leadership experience preferred. Â
- Prior program development experience preferred
- Experience in Electronic Medical Records and Microsoft Office products is required
- Strong interpersonal, communication, and leadership skillswith a demonstrated ability tocollaborate effectively across disciplines, and guide teams through periods of growth and change.
