Commonwealth Home Health Network is looking for Physical Therapists to join our team. CHHN provides skilled in-home, and unskilled services to seniors, older adults, and those who are disabled.

• Provide care for our community of recovering patients, ushering them back to a life of independence.
• Create and follow personalized plan of care in collaboration with RN and the interdisciplinary team.
• Educate our patients and their families on next steps in continuing care and improvement of day to day life including: therapy, exercise, and treatments. 
• Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.
• Focus on the goal of improving the overall health of our community.
• Oversee Physical Therapy Assistants. 
• Perform all duties with a passion for compassion.

Qualifications

• Valid Physical Therapy license (required)
• 1+ years of PT experience 
• A heart for providing the best care to our community
