We’re seeking a dedicated, full-time Hospice Registered Nurse Case Manager to walk alongside patients and families with skill and grace. This role has competitive pay ($70,000–$88,000 BOE) and a generous benefits package.

Hospice Registered Nurse Case Manager’s Responsibilities: (Included, but not limited to) 

  • Care for patients where they call home.
  • Use your clinical skills and emotional support skills to support patients and families.
  • Create care plans that treat the whole person—body, mind, and spirit.
  • Work closely with providers, referral sources, and our amazing interdisciplinary team (IDT).
  • Be a steady, calming presence during a difficult time—and help others feel just a little more at peace.
  • Educate families, empower caregivers, and be the go-to person when people have questions.

Requirements for the Hospice Registered Nurse Case Manager: 

  • Ability to work M-F 8:30am-5pm
  • On call needs as required to meet patient care needs (rotating nights, weekends, holidays)
  • Current Registered Nurse license in the state of Michigan
  • Hospice, home care, acute care, or case management experience
  • Confidence working independently—but also a team-first attitude
  • A big heart and the ability to keep calm under pressure
