About Rehab and Nursing Center-Kentridge 

We are located on a 10-acre campus. Our indoor and outdoor spaces are designed to provide opportunities for improving independence, strength, and confidence. Two main floor courtyards provide outdoor space for rest and renewal. Most patient rooms are private, and have an attached bathroom and shower, television with cable, phone, and a large window. Each unit has a large and accessible living and dining space, and a menu-based meal service is provided. 

 

Scope of work 

Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations. Demonstrates expertise, current knowledge in nursing care and management of a caseload of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment.

 

Qualifications

  • Bachelors Degree in Nursing or a healthcare related field Required
  • 2 years of relevant experience in clinical nursing or case management/managed care or related field Required
  • 5 years of relevant experience in clinical nursing or case management/managed care or related field Preferred
  • MI Registered Nurse (RN) Upon Hire Required
  • Certified Case Manager (CCMC) Upon Hire Preferred
