Position Summary
As a Home Health Aide, you will provide compassionate, personalized care that helps patients remain safe, comfortable, and independent in their homes. Working closely with our nursing team, you will follow individualized care plans while building meaningful relationships with the patients and families you serve.
Responsibilities
- Assist with personal care, including bathing, grooming, dressing, and hygiene
- Provide companionship and emotional support
- Prepare meals according to patient needs
- Perform light housekeeping and household tasks
- Observe and report changes in patient condition to the nursing team
- Maintain a clean, safe, and comfortable environment
- Follow each patient's individualized Plan of Care
- Complete other assigned duties related to patient care
Qualifications
Required:
- High school diploma or GED
- Valid driver's license
- Reliable personal transportation
- Ability to pass a criminal background check
- Authorization to work in the United States
Preferred:
- Previous experience as a Home Health Aide, Caregiver, Personal Care Aide, or Certified Nursing Assistant (CNA)
- Excellent communication and interpersonal skills
- Compassionate, dependable, and patient-focused attitude
