What You’ll Do:

  • Help residents adjust to their new environment with compassion and care

  • Develop social histories, psychosocial assessments, and discharge plans

  • Support residents and families during transitions and emotional challenges

  • Document progress notes and contribute to care plans

  • Foster a positive environment through volunteer programs and family group meetings

  • Uphold residents’ rights and ensure personalized attention to individual needs

 

What We’re Looking For:

  • Preferred: Bachelor’s degree in Social Work or related field

  • Healthcare experience highly preferred

  • Ability to connect positively with residents, families, and team members

  • Must meet applicable State and Federal requirements

  • Comfortable using basic computer systems and documentation tools
