Functional Medicine Nurse Practitioner / Physician Assistant
Location: Great Neck, NY
Schedule: Part-Time (up to 30 hours/week)
Compensation: $60–$80/hour (based on experience) + performance-based bonus
Injectables/Aesthetic Experience: Preferred, not required

The Role
We are seeking a Functional Medicine Nurse Practitioner or Physician Assistant who views healthcare as a partnership and medicine as an evolving pursuit. This role is ideal for a provider who values clinical depth, genuine connection, and long-term outcomes over quick fixes.
You will guide clients through personalized treatment plans, interpret advanced labs, and help them understand why they feel the way they do and how to improve it.

Key Responsibilities
- Conduct comprehensive exams, bloodwork reviews, and functional assessments
- Develop personalized treatment plans that may include:
- Lifestyle and stress management
- Supplement and peptide protocols
- IV therapy and regenerative services
- Hormone optimization (HRT)
- Medical weight loss
- Translate complex lab data into clear, actionable guidance for clients
- Collaborate daily with Client Advocates, RNs, front-of-house staff, and providers
- Maintain HIPAA compliance, safety standards, and infection control protocols
- Complete accurate, same-day EHR documentation
- Maintain active licensure, CE requirements, and compliance training
- Stay current with advancements in functional medicine, hormones, peptides, and longevity care
- Support training or shadowing during new ReLive center onboarding, as needed
Ideal Background
- Active, unrestricted NP or PA license in New York
- 3+ years of clinical experience in functional medicine or hormone optimization
- Experience or strong interest in regenerative medicine, peptides, and medical weight loss
- Comfortable learning new systems (EMR, lab portals, pharmacy platforms)
- Passion for helping patients reclaim their health with integrity and safety
- Aesthetic injectables experience (Botox, dermal fillers) is a plus
