Pay Range:
$34.21 - $51.31A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.

• Core Function: Rehab/Therapy/Sleep
• Schedule: PRN - one weekend a month, one holiday a year and looking for some availability during the week as well.

Education Qualifications
- Graduate (e.g., bachelors, master, or doctorate) from an accredited program which meets the licensure requirements in the state of practice is required.

Certifications, Licenses and Registrations
- IL: State Licensure as an Occupational Therapist is required OR has been authorized to practice by the Illinois Department of Professional Regulation (IDFPR) and must obtain licensure within 6 months.
- WI: State Licensure as an Occupational Therapist is required OR has been issued a temporary licensure to practice and must obtain licensure within 6 months.
- Certified Basic Life Support (BLS) or completion in the first 90 days of employment is required.
