Description

Position Summary:

Oversees all outstanding lab and diagnostic imaging results for all UCHC providers to ensure completion of ordered tests. Patient care coordination efforts as directed by Chronic Care Manager.

Essential Functions

  1. Utilizes the nursing process in the administration of direct nursing care to patients in accordance with nursing/clinic policies, procedures and standards according to the Arizona State Practice Act and other regulatory agencies.
  2. Documents procedures, medications, delivery of care, and pertinent information regarding nursing care rendered to the patient. Reports observed changes in the patient’s physical and psychological needs and maintains accurate up to date records.
  3. Maintains Patient confidentiality and protects operations by keeping information confidential according to UCHC and HIPAA policies, procedures and regulations.
  4. Obtains data necessary to deliver quality patient care from patient history, vital signs, charts, interviews, observations, physical assessment, etc., while assuring the safety, comfort and well being of the patient.
  5. Complies with quality assurance, safety, infection control and emergency procedures consistent with clinic policy and procedures to ensure maintenance of a safe working practice for patient and colleagues.
  6. Maintains safe and clean working environment by complying with procedures, rules, and regulations. Protects patients and employees by adhering to policies and procedures related to infection control, medication administration, storage procedures and controlled substance regulations.

Additional Duties & Responsibilities

  1. Attends and participates in staff meetings and in in-service and outside training as appropriate or required and completes personal and professional development plan items to maintain skills and develop personal growth through training and education.
  2. Participates in inter-disciplinary task forces and work groups as relevant.
  3. Assists in orientation and training of new staff and/or student externs as appropriate.
  4. Participates in and complies with Quality Improvement initiatives and on-going continuous improvement efforts as appropriate.
  5. All other duties as assigned.



Requirements

Qualifications/Requirements

  • Graduate of an accredited school of nursing.
  • Three years of professional nursing experience required.
  • Bilingual in English and Spanish preferred.
  • Reliable vehicle and proof of insurance

Licensure/Certification

  • Current license to practice as an RN (Registered Nurse).
  • Current CPR certification
  • Valid Arizona driver’s license.

Experience/Skills

  • Demonstration of clinical skills and competencies as described in UCHC’s Clinical Competency assessment
  • Knowledge of nursing policies, regulations and procedures to administer patient care (and by end of Introductory Period, knowledge and fluency in UCHC patient care policies and procedures).
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Demonstrated use and knowledge of medical equipment and instruments to administer patient care
  • Knowledge of common safety hazards and precautions to establish a safe working environment.
  • Able to identifying problems and recommending solutions.
  • Skill in preparing and maintaining medical records and responding to correspondence.
  • Skills in establishing and maintaining effective working relationships with patients, medical staff and the public.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to communicate clearly verbally and written.
  • Ability to handle multiple tasks efficiently in a busy environment.
  • Ability to adjust to changing situations and needs.
