Thriving Together Support Services in Greenfield, IN is looking for compassionate and dedicated Home and Community Support Professionals (HCSPs) to join our growing team! In this rewarding role, you'll make a meaningful difference in the lives of individuals who need support, helping them thrive in their daily lives.Â

Responsibilities:
- Provide direct care and personalized support to individuals with disabilities or special needs
- Assist clients with daily living activities including personal hygiene, meal preparation, and mobility
- Implement individualized support plans to promote independence and quality of life
- Transport and accompany clients to appointments, community outings, and activities
- Document care activities, observations, and progress accurately and in a timely manner
- Communicate effectively with clients, families, and care team members
- Foster a safe, supportive, and positive environment for all individuals served
Requirements:
- Previous experience as a DSP, HCSP, caregiver, or in a direct care role preferred
- Genuine compassion, patience, and dedication to supporting others
- Ability to pass a background check and meet state compliance requirements
- Valid driver's license and reliable transportation preferred
- Strong communication and interpersonal skills
- Ability to work independently as well as collaboratively with a care team
- Flexibility to work various shifts including evenings, weekends, or holidays as needed
