The primary purpose of this position is to serve as a Community Health Outreach Worker for the Guilford County Formerly Incarcerated Transition Program. Works individually with participants to connect them with health information and health care services, while assisting in the navigation of these services. Identifies and assists participants in addressing the social drivers of health such as: housing, employment, food, and other barriers to successfully re-engaging in society after incarceration. This position works very closely with various community agencies and Health and Human Services organizations.


Special Requirements: 

History of incarceration and successful reentry into the community after at least three-years from release. Requires a valid North Carolina driver’s license with a three-year acceptable driving record.

 

Responsibilities: 

  • Connects residents with community resources that offer educational programming and other supportive services. 
  • Understands and educates participants on medication access options and services. 
  • Assists in provider-patient care relationship building. 
  • Helps participants understand how to use resources to find health coverage or apply for assistance when uninsured. 
  • Service locations for the position are clinics, home visits, transition centers or other community settings. 
  • Participates in the Transition Care clinic network conferences, trainings and meetings.
  • Participates in NC FIT Program phone call, webinars and meetings.
  • Participates and maintains required trainings to engage the population 
  • Represents the organization at local community meetings including but not limited to: Guilford County Re-entry Council, NC Works Partners Meeting, Guilford Community Care Network, Housing coalitions, Partners Ending Homelessness, etc. 

 

Requirements & Experience:

Requires good written and oral communication skills, ability to work well independently and with a team and basic computer/word processing skills. Previous experience in peer support work desired. Some early morning, evening, and weekend work may be required. Demonstrated possession of knowledge, skills, and abilities as stated above through at least one year of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, prison or other social or rehabilitation services.

Education:

High school diploma or GED equivalent preferred. Previous experience in peer support work desired
