Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence. You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.

Key Responsibilities

  • Perform patient evaluations and develop individualized treatment plans
  • Provide therapy to improve strength, balance, mobility, and function
  • Monitor patient progress and adjust treatment plans as needed
  • Maintain accurate and compliant clinical documentation
  • Communicate effectively with physicians, residents, families, and care team members
  • Participate in care planning and discharge planning
  • Provide education and guidance to residents and staff

Qualifications

  • Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
  • Active, unencumbered Physical Therapist license in South Carolina (required)
  • Must maintain all continuing education and remain in good standing with the State Board
  • Experience in a skilled nursing or rehab setting preferred
  • Familiarity with Net Health and PointClickCare (PCC) preferred
  • Strong clinical, communication, and organizational skills
  • CPR certification (if required by licensure)
