Position Summary

The Hospice Nurse Practitioner (NP) provides compassionate, expert clinical care to patients facing life-limiting illness in both home and inpatient hospice settings. Working closely with the Medical Director and interdisciplinary team, the NP focuses on comfort, symptom management, and honoring each patient's goals of care.

This role blends strong clinical skills with a heart for service. The NP plays a vital role in ensuring that our patients experience dignity, comfort, and support wherever they call home.

 

What You Will Do

Community & Home Visits

  • Visit patients in private homes, assisted living, and nursing facilities
  • Perform comprehensive assessments and manage pain and complex symptoms
  • Prescribe medications and treatments aligned with hospice goals
  • Complete required hospice face-to-face visits and recertifications
  • Collaborate with RN case managers and the interdisciplinary team
  • Support patients and families through education and goals-of-care conversations

Inpatient Hospice Facility

  • Provide daily rounding for patients receiving General Inpatient (GIP) or Respite care
  • Manage acute symptom crises requiring short-term intensive support
  • Communicate closely with families about patient status and care plans
  • Assist with admissions and transitions between levels of care

Team & Mission Support

  • Participate in Interdisciplinary Group (IDG) meetings
  • Provide guidance to staff on complex symptom management
  • Maintain timely, accurate documentation in accordance with hospice regulations
  • Support quality improvement and compassionate standards of care

 

Qualifications

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Current, unrestricted NP license in the state of practice
  • DEA registration and board certification (Family, Adult-Gerontology, or Acute Care NP)
  • Strong communication skills and comfort with serious illness conversations
  • Valid driver's license and reliable transportation

Preferred qualifications:

  • Experience with and passion for hospice and palliative care
