Sonic Summary

We are looking for a compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide skilled nursing care in a Home Health setting.
This position focuses on delivering high-quality, patient-centered care to support patients and their families in promoting recovery and maintaining health.
The LPN/LVN will work under the supervision of a Registered Nurse and follow physician orders as well as individualized care plans.
Candidates should have at least one year of experience in a nursing role to qualify for this position.
The salary for this role is competitive and commensurate with experience.

We are seeking a compassionate and qualified Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in a Home Health setting. This role supports patients and their families in the home by delivering high-quality, patient-centered care aimed at promoting recovery, maintaining health, and preventing hospitalization. The LPN/LVN provides nursing services under the supervision of a Registered Nurse and in accordance with physician orders and the individualized plan of care developed by the interdisciplinary care team.
