We are seeking a skilled and compassionate Occupational Therapist to join a Skilled Nursing Facility in Gridley, California. This role focuses on helping patients regain independence in daily living activities following illness, injury, or surgery. You will work with a geriatric and post-acute population, developing personalized treatment plans that improve functional ability and overall quality of life. Job Responsibilities:
• Evaluate and treat patients with physical, cognitive, and functional limitations in a Skilled Nursing Facility setting
• Develop and implement individualized occupational therapy treatment plans
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding
• Recommend adaptive equipment and environmental modifications to improve independence
• Monitor patient progress and adjust treatment plans as needed
• Maintain accurate and compliant documentation of patient care and outcomes


Requirements

• Active state license as an Occupational Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recent OT experience preferred, ideally in a SNF or geriatric setting
• Strong problem-solving and patient care skills
• Ability to work collaboratively within a multidisciplinary team
