Job DetailsLevel: ExperiencedJob Location: Grosse Pointe Woods, MI 48236Job Shift: DayThe Rivers - Grosse Pointe is seeking a candidate to join the team as a Social Services Tech. As a member of the interdisciplinary team, the Social Services Tech provides services to respond to the emotional needs of residents and their families, as well as assessment, documentation, and reporting responsibilities. In part, the responsibilities include: Work with the Social Service Team, starting with the Admission Process, meet with the resident and family, assist in completing admissions paperwork and conduct evaluations and assessments, including identification of resident needs. Provide/arrange for social work services as indicated by resident/family needs, transfers, and changes in care as requested Maintain a current list of community resources for effective and efficient discharge planning. As a team, implementing interventions that achieve treatment goals, address resident needs, and link social supports, physical care, and physical environment to enhance quality of life. Assist the team to complete the MDS process in a timely manner and maintain/update care plans as needed including assist to coordinate family meetings and facilitate such meetings to ensure residents and families are kept informed of condition changes and interventions. QualificationsIdeal Candidates: Must be a HS Graduate or equivalent, Associates / Bachelor’s degree, preferred, or working toward licensure in Social Work, or related field, preferred Have One-year of social work experience, preferred, working with individuals in a long-term or sub-acute care rehabilitation facility, or work as a discharge planner in a healthcare setting Always demonstrate honesty and integrity in all responsibilities. Possess assessment skills sufficient to evaluate residents’ history, behavior, and data collected to evaluate psychosocial needs, risk factors and residents’ responses to interventions. Be able to interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment and respect for residents. Demonstrate the highest level of interpersonal skills, including communication, empathy, sensitivity, and appropriate confidentiality to productively interact with residents, families, community, and interdisciplinary staff. Ensure that the quality and appropriateness of resident care meets or exceeds company and industry standards, and that all services are in compliance with state, federal, legal, regulatory, accreditation, and reimbursement guidelines. Contribute to quality improvement that increases the resident’s quality of life and raises the standards of the organization.   This can be a full-time benefits eligible position, or Part-Time position for the right candidate.    Because of the nature of the work, this position requires Mon-Fri Day shift hours availability, with weekends and evenings occasionally necessary, so having some flexibility with scheduling and availability is appreciated.
