The Intake Coordinator manages program admissions for individuals with intellectual and developmental disabilities. The Intake Coordinator will be the point person ensuring all required processes are complete for new individuals to begin receiving supports. This person will work across all programs. The priority will be at the discretion of the Chief Program Officer.


  • Manage the admissions and discharge process for individuals receiving supports and services including completing and maintaining all intake/discharge documentation. This will require understanding each program and necessary admission/discharge requirements
  • Knowledge of Medicaid, Medicare, SNAP and Social Security benefits and applications/certifications for each
  • Primary contact for referrals through OPWDD, Care Managers and families
  • Gather necessary documents for all new admissions
  • Participate/coordinate trial visits and pre-placement meetings for all new admissions
  • Ensure changes to individuals service authorizations and amendments are submitted to OPWDD
  • Collaborate with the finance department and benefit specialists to troubleshoot issues impacting billing and cash flow
  • Liaison with Care Managers and Regional Office for obtaining eligibility
  • Attending Residential Opportunity Committee meetings in addition to the DRO Day Service Providers meetings
  • Develop and maintain an internal system for monitoring enrollment progress. Give reports as necessary
  • Coordinate all Internal Movement
  • Ensuring all Medicaid and waiver documentation is in place prior to admission
  • Submit documentation for enrollment in NetSmart (if required by the program)
  • Establish and maintain professional relationships with key employees involved in Residential Services, Day Habilitation, Community Living as well as, external community professionals
  • Attend all meetings as assigned by the Associate Executive Director
  • This position requires flexibility as hours may vary.
  • Participate on cross-functional committees when requested
  • Complete other duties as assigned

Requirements

  • High School Diploma required
  • Associates or Bachelor's Degree Preferred
  • At least 2 years working with individuals with intellectual disabilities
  • Valid NYS driver’s license
  • Complete all state and agency required training
  • Excellent verbal, written, interpersonal and communication skills
  • Knowledge and understanding of local and regional regulatory operations
  • Knowledge and understanding of OPWDD regulations
  • Understanding and commitment to community-based support for persons with disabilities
  • Experience with intake and admissions
  • Excellent time management skills
  • Proficient in Microsoft Office (Outlook, Word, and Excel)
