The Population Health Scheduler is a full-time, day-shift position within the Population Health call center, responsible for supporting care coordination through proactive patient outreach, scheduling, and education. The ideal candidate is self-motivated, organized, and able to work independently while managing daily outreach volumes and meeting productivity expectations in a fast-paced environment.  Previous healthcare, scheduling, or call center experience is preferred, but candidates with strong communication skills and a passion for patient engagement are encouraged to apply.

 

The Population Health Scheduler oversees annual wellness and preventative visit scheduling, maintains precise patient data and electronic health records, conducts population health outreach through various channels, addresses general inquiries, verifies insurance benefits, ensures timely payments, coordinate schedules, aids patient in navigating healthcare system and directs them to relevant departments for necessary information and care.

Responsibilities

  • Coordinates and Schedules Annual Wellness/Preventive appointments
    • Schedules appointments for Population Health Navigators and Primary Care Providers
    • Verifies insurance benefits, frequency, and eligibility for wellness appointments
    • Provides education specific to population health measures and wellness appointments.
    • Collaborates with interdisciplinary staff to coordinate and schedule for Providers and navigators
    • Cancels and reschedules patients as requested by patient, provider, navigator, or staff
    • Consistently reviews all Population Health Navigator schedules to ensure appropriate number of patients scheduled per day
    • Monitors multiple message pools to ensure continuity of care for patient population
  • Maintains electronic medical records by entering accurate patient demographic, insurance, and scheduling information
    • Enters and verifies all required demographics are current and up to date to ensure accurate electronic health record
    • Verifies patient billing information and correct contact information
    • Communicates with all clinics using the messaging system in the electronic health record
  • Schedules on demand virtual telehealth appointments
    • Maintains the accuracy of the on-demand provider schedule
    • Secures appropriate payments for on demand telehealth appointments
    • Communicates with on-call provider to ensure a warm transfer for a seamless transition
  • Participates as a member of the team to achieve business goals, quality outcomes, and customer satisfaction
    • Assists with providing access to primary care providers
    • Answers general patient questions related to all operations of the ambulatory clinics
    • Maintains up to date information on clinic hours of operations and updates patients of any scheduling concerns
    • Communicates change in clinic hours when necessary
    • Regular and timely attendance is required for this job

Qualifications

Education:

  • High School or GED or equivalent

Experience:

  • One (1) year of clerical experience or clinical role in healthcare setting

Skills:

  • Excellent verbal and written communication skills
  • Multi-tasking, time-management, and organization
  • Continuous observation of a computer monitoring and working with frequent interruptions and rapidly changing priorities
