Job Description:

Liquid Personnel is recruiting a Social Worker for its client’s Integrated Learning Disabilities Team based in Hackney.
The role involves providing high-quality, person-centred social work support to adults with learning disabilities within an integrated health and social care team. It focuses on promoting independence, safeguarding, and improving outcomes through collaborative working with health professionals, families, and community services.

What will your responsibilities be?

  • Undertake assessments under the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation
  • Develop and review care and support plans that promote independence, choice, and wellbeing
  • Work collaboratively with health colleagues such as nurses, psychologists, and occupational therapists to deliver integrated care
  • Safeguard adults at risk by identifying concerns, conducting enquiries, and implementing protection plans
  • Support individuals through transitions, including moving from children’s services to adult services
  • Maintain accurate and timely records using electronic case management systems
  • Participate in multi-disciplinary meetings, case conferences, and best interest decisions
  • Advocate for service users and ensure their voices are central to decision-making
  • Contribute to service development and continuous improvement initiatives

Qualifications & Experience:

To be successful in this role you must have, 

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
