Help Individuals Find Stability Through Housing

The Hampton-Newport News Community Services Board (H-NNCSB) is seeking a compassionate Housing Stabilization Specialist to help adults with mental health, developmental disabilities, substance use disorders, and co-occurring conditions obtain and maintain safe, stable housing.

In this rewarding role, you will partner with individuals who who are homeless, have past incarcerations, may live in group homes, and/or have frequent psychiatric hospitalizations. Your work will empower clients to achieve greater independence while improving their quality of life.

Required Qualifications:

Bachelor's Degree in Human Services such as Social Work, Psychology, Sociology or Counseling.
One (1) year case management experience including housing resource for adults with mental health, developmental disabilities, substance use disorders and co-occurring disorders.

What You'll Do

As a Housing Stabilization Specialist, you will provide case management and housing support services that promote long-term housing success.

Responsibilities include:

Assess clients' housing needs and develop individualized housing stabilization plans.
Assist individuals in locating, securing, and maintaining safe and affordable housing.
Conduct home and community visits to monitor housing stability and provide ongoing support.
Teach independent living skills, including budgeting, tenant responsibilities, and accessing community resources.
Coordinate services with landlords, property managers, behavioral health providers, social service agencies, and community partners.
Maintain accurate documentation, service plans, and progress notes in the Electronic Health Record (EHR).
Advocate for clients while helping them overcome barriers to housing and community integration.
