Franklin County Public Health is seeking a compassionate, organized, and dedicated Registered Nurse (RN) to provide high-quality public health and home health nursing services throughout Franklin County. This position offers the opportunity to make a meaningful impact on the health and well-being of individuals and families within our communities.

Position Summary

The Registered Nurse provides skilled nursing care and public health services in clients' homes and community settings. The RN works collaboratively with clients, families, healthcare providers, and community partners to promote health, manage chronic conditions, support recovery, and improve overall quality of life.

Essential Duties and Responsibilities

  • Provide skilled nursing assessments and interventions in patients' homes.
  • Develop, implement, and evaluate individualized care plans.
  • Administer medications, treatments, and other physician-ordered services.
  • Monitor patient progress and communicate changes in condition to physicians and other healthcare providers.
  • Educate patients and families regarding disease management, medications, treatment plans, and health promotion.
  • Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
  • Participate in public health programs and initiatives as assigned.
  • Coordinate care with interdisciplinary team members and community resources.
  • Maintain patient confidentiality and comply with HIPAA regulations.
  • Attend required meetings, trainings, and continuing education activities.

Qualifications

Required

  • Current Iowa Registered Nurse (RN) license in good standing.
  • Valid driver's license.
  • Reliable personal vehicle for travel throughout Franklin County.
  • Current automobile insurance meeting state requirements.
  • Excellent communication, organizational, and critical thinking skills.
  • Ability to work independently and manage a flexible schedule.

Preferred

  • Home health, public health, community health, or case management experience.
  • Experience working with diverse patient populations.
