Community Health Worker

Location: Hannibal, MO

Department: Clarity Healthcare

Employment Type: Full-Time

Shift: Monday - Friday

Job Summary:

The Community Health Worker (CHW) serves as a vital bridge between individuals, healthcare providers, and community resources. This role supports overall community health by helping patients reduce risk factors, navigate healthcare and social service systems, and connect with culturally appropriate resources. CHWs empower individuals to take an active role in their health while strengthening the community’s capacity to thrive.

Key Responsibilities: 

  • Facilitate communication between patients and healthcare/social service systems, promoting patient empowerment and understanding.

  • Identify and troubleshoot barriers that may prevent patients from adhering to care plans.

  • Conduct community outreach to identify, recruit, and engage high-risk individuals.

  • Serve as a liaison between healthcare/social services and the community to improve service access, quality, and cultural responsiveness.

  • Support the primary care team with care management processes, including guideline-based care, disease management, case management, and chronic disease self‑management education.

  • Increase health knowledge and self‑sufficiency through community education, advocacy, and social support efforts.

  • Collaborate with patients and care teams to develop solutions that overcome barriers to achieving healthcare goals.

  • Connect patients with community resources and appropriate health center services.

  • Provide follow-up support through phone calls, home visits, and outreach in community settings.

  • Assist with general support activities and special projects as assigned.

  • Guide individuals through reviewing coverage options, completing eligibility forms, and enrolling in QHPs.

  • Deliver information in ways that are culturally and linguistically appropriate for the populations served.

Knowledge, Skills, and Abilities:

  • Understanding of harm reduction principles, motivational interviewing, and other relevant treatment modalities.

  • Knowledge of local community culture, values, and available community resources.

  • Strong customer service principles, including needs assessment, quality service delivery, and follow‑up.

  • Active listening skills and the ability to understand patient perspectives.

  • Clear verbal communication skills, with the ability to explain information in a way that is easy to understand.

  • Proficiency in Microsoft Excel.

  • Excellent written and verbal communication skills.

Experience and Education Qualifications:

  • Preferred: Bachelor’s degree in a related field from an accredited college or university.

  • Required: High school diploma or equivalent.

  • Preferred Experience: Two (2) years of experience in community health, human services outreach, or healthcare.

  • Additional Valuable Experience: Customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, and public speaking.

Employment Requirements:

  • Must provide evidence that incumbent is free of infectious and contagious disease prior to beginning employment and annually thereafter.

    • TB– Expected for all direct care staff. 

  • Current driver’s license, acceptable driving record and current auto insurance.

  • Achievement of the following training:

    • CHW training program at one of the recognized curriculum providers.

    • Education on the Missouri Foster Care program.

    • Trauma-informed training, to better understand/address foster care experiences.

    • Training on common behavioral health conditions in children.

    • Training on suicide prevention and substance use.

    • First Aid certification within three (3) months of beginning employment and upon renewal date.

    • Obtain CPR certification within three (3) months of beginning employment and upon renewal date.

  • Behavioral Management training completed within thirty (30) days of employment or first available class after start date:

    • CPI for Residential programs

    • Peaceful Intervention or Verbal De-escalation for Outpatient programs

  • Completion of “Direct Service Expectations” within 30 Days of employment
