GENERAL PURPOSE OF JOB:

As a Care Coordinator, this individual will be a highly motivated, quality-conscious healthcare worker interested in working in a fast-pace environment where one-on-one time with patients and positive outcomes are the ultimate measurement of success. 

KEY RESPONSIBILITIES: 

  • Assists patients with check in process, record updates on medications/allergies, etc., and ensure vitals are collected.
  • Ensures accurate and timely documentation into the EMR; i.e.- patient interviews and histories, vital signs, treatments, test results. 
  • Provides patients with instructions and other information regarding preparations for the service and necessary registration. 
  • Maintains confidentiality of all patient and organization information and follows federal and state regulations and organization policies regarding protection of patient information.
  • Participates in development activities and maintains affiliations with appropriate associations, as applicable. 
  • Supports preventative healthcare initiatives and promote a team-based approach to patient care.
  • Coordinates and manages all aspects of a patient’s healthcare, including scheduling and treatment plans.
  • Serves as the primary point of contact for providers, patients, facilities, and other healthcare professionals.
  • Performs post-visit and between-visit tasks, such as patient outreach, referral management, follow-up calls, and appointment scheduling, as directed by the Health Center Manager or provider.
  • Processes provider orders, requests and reviews medical records, and manages faxes/attachments.
  • Collaborates closely with Operations Team and external vendors to leverage program benefits that enhance patient outcomes. 

JOB REQUIREMENTS:

Education: 

  • Graduate of accredited school of medical assisting, or combination of equivalent education and experience, OR, if LPN, graduate from an approved nursing program with an associate degree or equivalent.

Experience: 

  • Minimum of three (3) years of experience as a Medical Assistant (MA) or Licensed Practical Nurse (LPN).
  • Experience working in a Primary Care setting. 
  • Prior experience in Care Coordination preferred.
  • Bilingual in Spanish a plus. 
  • Experienced in or a strong passion for delivering patient-centered care. 

Certificates, Licenses, Registrations: 

  • Current CPR (BLS) certification or must be obtained within 60 days of hire.   
  • If LPN, current license in state of residence or compact licensure.
  • Medical Assistant certification through AAMA, AMT, NHA or NCCT. Certification through another accredited association will need review/approval.

Knowledge, Skills & Abilities: 

  • Proficiency in using technology and various applications to provide patient care both in person and virtually. 
  • Excellent verbal and written communication skills. 
  • Strong organizational skills with the ability to set priorities and work independently. 
  • Commitment to preventative healthcare programs and a team-based approach to care delivery.
  • Exceptional interpersonal skills with the ability to build rapport and effectively communicate with patients and healthcare professionals.
  • Proven ability to work effectively with diverse populations and a demonstrated commitment to fostering inclusion.
  • Understanding of medical terminology, patient care procedures, and healthcare documentation best practices. 
  • Ability to assess patient needs, coordinate care efficiently, and address challenges proactively. 
  • Capacity to manage multiple tasks efficiently in a fast-paced environment while maintaining high-quality patient care. 
  • Ability to work effectively with healthcare providers, patients, and external partners to optimize patient outcomes. 
  • Flexibility to adjust to evolving healthcare environments, technology, and patient needs.  
  • Commitment to supporting patients in navigating their healthcare journey, ensuring access to necessary resources and services.
