Job Summary

The Certified Diabetes Educator – Registered Nurse (RN) plays an important role in supporting Hamilton Health Center’s comprehensive approach to diabetes care and patient education. This position is available on either a full-time or part-time basis, with flexible scheduling options.

The Certified Diabetes Educator provides individualized diabetes education, self-management support, and ongoing guidance to patients and their families in a patient-centered healthcare environment.

The Diabetes Educator assesses patients’ educational and diabetes management needs, develops individualized care plans, and provides education related to healthy lifestyles, blood glucose monitoring, medication management, risk reduction, insulin administration, and insulin pump therapy. This position works collaboratively with physicians and other healthcare professionals to support patients in effectively managing their diabetes and achieving improved health outcomes.

Essential Functions

  • Assess patients’ diabetes education and management needs and develop individualized care plans.
  • Provide diabetes self-management education in accordance with applicable national standards.
  • Educate patients, family members, and significant others regarding healthy lifestyles and diabetes management.
  • Provide education regarding blood glucose testing and monitoring.
  • Educate patients regarding diabetes medications and medication management.
  • Provide education and instruction regarding insulin administration and insulin pump therapy.
  • Educate patients regarding risk-reduction strategies associated with diabetes.
  • Provide ongoing education and support to patients regarding diabetes self-management.
  • Review home glucose-monitoring records and monitor patients’ glycemic control.
  • Review diabetes medications and consult with physicians regarding patient care as appropriate.
  • Adjust medication and insulin doses in accordance with established and approved protocols when appropriate.
  • Accurately document patient interactions and maintain appropriate documentation for diabetes education services.
  • Support appropriate and complete billing documentation for diabetes education services.
  • Participate in organizational efforts to achieve and maintain applicable recognition standards for diabetes education programs.
  • Provide diabetes education training and updates to Hamilton Health Center staff.
  • Collaborate effectively with physicians and other healthcare professionals involved in patient care.
  • Maintain patient confidentiality and exercise appropriate professional judgment.
  • Comply with Hamilton Health Center policies, clinical procedures, and applicable regulatory standards.
  • Perform other assigned job-related duties as required.

Requirements:

  • Registered Nurse: Current and unrestricted Registered Nurse (RN) license.
  • Clinical Experience: Minimum of two (2) years of clinical experience.
  • Diabetes Care Knowledge: Strong knowledge of diabetes education principles and patient self-management practices.
  • Medication Management: Knowledge of diabetes medications, blood glucose monitoring, insulin administration, and insulin pump therapy.
  • Patient Education: Ability to assess patients’ educational and diabetes management needs and develop individualized care plans.
  • Communication Skills: Ability to communicate clearly and effectively with patients, families, physicians, and other healthcare professionals.
  • Interpersonal Skills: Ability to establish effective working relationships with individuals from diverse backgrounds.
  • Collaboration: Ability to work effectively as part of an interdisciplinary healthcare team.
  • Documentation: Ability to accurately document patient interactions and complete required clinical and billing documentation.
  • Professional Judgment: Ability to maintain confidentiality and exercise sound professional judgment.

Preferred Qualifications:

  • Bachelor’s Degree in Nursing or a related healthcare field.
  • Certification in diabetes care and education.
  • Previous experience providing diabetes education or diabetes self-management support.
  • Experience working in an outpatient, community health, FQHC, or similar patient-centered healthcare environment.
  • Bilingual in Spanish, Nepali, or French.
