Job DetailsJob Location: Harrisburg - Harrisburg, PA 17110Position Type: Full TimeSalary Range: $55,000.00 - $70,000.00 Salary/yearTravel Percentage: NoneJob Shift: DayDescription: The pharmacy coordinator is to assist the clinical operations to ensure that medications are available for patient appointments and to help patients overcome medication access barriers. The Pharmacy Coordinator/LTC focus on the inventory management of clinic administered drugs, reducing prescription drug costs through manufacturer copay cards and patient assistance applications, with a focus on hard-to-reach patients, those patients that have been missing for laboratory work and patients late for medication pickups and administration. The Pharmacy coordinators also work to assist patients with transportation access and decreasing food insecurities via the management and reporting of these non-profit resources. General Duties and Responsibilities: ● Manage and Document Clinic administered drugs including but not limited to the medication wall and long acting injectables. ● The Long acting Injectables - ● Manage the receipt and ordering of long acting injectables to the clinic from our specialty pharmacy partners by following and documenting in the LAI database. ● Reach out to patients on LAI to remind them of upcoming appointments and follow up for late injections based on the time before or after the targeted treatment dates as evidenced in the LAI data base, ● Record new or discontinued prescriptions in the LAI database. ● Review clinic Schedule for the following day to prepare for the day/week to ensure the injections are in the clinic for the upcoming targeted treatment windows (Sublocade, Vivitrol, E, T, Ozempic, Apretude/ Cabenuva ) for the day/week appointments. ● Record daily temperature to ensure integrity of refrigerated medications. ● Receive and document medications received to the clinic by the partner pharmacies in the appropriate database daily and safely secure medications. ● Contact patients who have requested to pick up medications at the clinic or utilize the Medication wall. ● Review and submit manufacturer copay and patient assistance applications, recording card IDs in documentation of the actions in the EMR. ● Communicate with drug manufacturers regarding the applications and verification of injection dates etc. and record information in the EMR as applicable. ● Monitor Buy and Bill Clinic Administered Drugs - for in stock administration and appropriate insurance billing, validate correct procedure codes and notify biller of submission. ● Communicate patient medication access issues to providers such as pharmacy lock ins so that prescriptions can be rerouted appropriately. ● Monthly audit clinic medication stock for outdated items, items for destruction and or inventory replenishment and document accordingly in the database. ● Weekly audit inventory and reorder as needed to maintain Par levels. ● Record the receiving of the Sublocade prescriptions in the database base according to the sublocade REMs and DEA requirements. ● Monitor Pharmacy communication channels and take appropriate actions. ● Respond to EMR messages sent by providers to reach out to patients to schedule appointments based on their review of refill requests - ● Monitor Updocs for Patient response to messages sent` ● Monitor and Respond to Conversation messages in the patient communication app. ● Manage the inventory of transportation assistance and giant eagle gift cards by ensuring the receipt of the assistance is recorded and documented on the appropriate forms to return to the nonprofit and document the receipt in the database. ● As needed participate in the data collection and documentation for population studies and grants such as the Cabenuva study. ● Replenish pertinent applicable pharmacy related patient information for the clinic rooms such as MMJ contracts, Medication receipts, PAP applications on a regular basis. ● Medication Inventory and Storage (Non-Controlled) β—‹ Manage and document clinic-administered drug inventory β—‹ Receive and document non-controlled medications received from partner pharmacies in the appropriate database daily β—‹ Safely secure non-controlled medications in the designated storage areas β—‹ Record daily refrigerator temperature to ensure integrity of refrigerated medications; report excursions immediately β—‹ Conduct monthly audit of clinic medication stock for outdated items, items for destruction, and inventory replenishment; document accordingly β—‹ Conduct weekly inventory audit and reorder as needed to maintain par levels β—‹ Replenish pharmacy-related patient information materials in clinic rooms ● Any other duties as assigned.
